Getting Started

Getting started with medTRANS is simple. Complete the RFP Questionnaire and indicate whether you are requesting an Illustration, Indication, or Proposal. Each tier requires a different level of census and claims information, which is outlined in the questionnaire. Once your completed questionnaire and supporting documentation are received, our underwriting team will review your submission.

RFP Submission Process

  1. Complete the RFP Questionnaire
    • Download the fillable RFP Questionnaire and provide the requested information about the employer group.
    • Complete all required fields before submitting the questionnaire.
    • IMPORTANT: If you are requesting underwriting for multiple employer groups, a separate RFP Questionnaire and email are required for each group. Please don’t include multiple employer groups in a single submission.
  2. Submit the Questionnaire and Non-HIPAA-Sensitive Files
    • Once the questionnaire is complete, click “Email completed form to CCMS” at the bottom of the PDF. An email will open with a pre-populated subject line. Please do not alter the subject line.
    • Attach all required non-HIPAA-sensitive files to the email and send.
      Having trouble submitting? Make sure all required fields in the questionnaire have been completed.
  3. Send HIPAA-Sensitive Files Securely
    • Send all required HIPAA-sensitive files in a separate, secure email to rfp@completecaptive.com.
    • Do not include these HIPAA-sensitive files with the email generated by the RFP Questionnaire.

Underwriting Guidelines & Resources

These resources outline the guidelines, data requirements and methodology used by medTRANS in the underwriting process. Requirements vary depending on whether you are requesting an Illustration, Indication or Proposal.

  • Illustration – Based on 100% manual rates and provides a conservative estimate.
  • Indication – A non-binding estimate that is generally close to a final offer.
  • Proposal – A binding offer between the insurer and the insured.
